First month free for New York firms onboarding this quarter.

New York Medicaid practice software

Community and Chronic Care Medicaid applications, from intake through HRA submission.

MedicaidHQ collects the client record, reconciles the five-year lookback, produces the NYS forms, and assembles the packet the way New York City HRA expects to receive it. Built for elder law practices doing Medicaid work — from solo attorneys to multi-attorney firms.

$249/month, billed monthly. No implementation fee.

Illustrative — not a real matter.

What the system handles

One record per matter. Every artifact — intake, documents, analysis, forms, signatures, retainer — lives against it, so nothing is reconstructed from email at filing time.

01

Structured intake

A questionnaire built around the NYS application, not a generic form. Answers populate the forms directly and flag missing income, resource, and transfer history as they are entered.

02

Document checklist

The required document set for the case type, tracked per item with received dates, expirations, and what is still outstanding from the client or the facility.

03

Lookback analysis

Uploaded bank statements are read and reconciled across sixty months, with transfers, cash patterns, recurring payments, and statement gaps surfaced for attorney review.

04

18+ NYS forms

DOH-4220, DOH-5178A, supplement A, MAP forms, and the balance of the NYS set generated from one record, against the current DOH revision of each.

05

HRA submission

Packet assembly and routing to the correct HRA destination for community and chronic care cases, with a filing record and confirmation retained on the matter.

06

Portal, signature, retainer

A client portal in your firm's name, e-signature with audit trail, and retainer agreements executed and stored against the matter.

The five-year lookback

Sixty months of statements, read line by line.

Statements are parsed per account and reconciled across the whole lookback period. Transfers, unusual cash activity, recurring payments, and gaps in the record are surfaced together rather than found one statement at a time.

What comes back is a worklist: every item an examiner is likely to ask about, with the account and date attached, ready for you to explain or document.

Attorney review is required. MedicaidHQ does not determine eligibility and does not file on its own.

Where generic legal software stops, and we don't

Practice management software will hold your documents. It will not tell you which HRA destination a chronic care application goes to, what order the packet has to be in, or which certification the facility has to supply before it will be accepted.

Those details are the difference between an application that is processed and one that comes back. They are encoded here, from HRA's own alerts and directives, and updated as those change.

A portal in your firm's name

Your clients sign in at your own address and see your firm, not ours. Documents, forms to sign, and what is still outstanding are all in one place.

Signature and retainer

Retainer agreements are generated on your letterhead and executed in the portal, with an audit trail kept against the matter.

Coordinators welcome

If you are a Medicaid coordinator working for several firms, each matter records its own attorney, and that is what the client sees.

Run one matter through it before you commit.

The first month is free, and your data is exportable at any point during it.
